Wow... We used to use (a permutation of (http://www.bbchatter.org/archive/files/reply.form.html)) that form on bb.chatter on the VAX at JMU. We called it the "Standard Reply Form," or SRF.
It *has* to be older than that, because I'm sure it was used @JMU longer than 1992, as 1) We used a permutation of the form, and 2) I don't know anyone who read a.f.w. I bet dwenius/plord could speak to this.
I'll see what I can do. I queried some JMUVAX oldbies, and they suggest that the form's been used there since 1990 or so. High probability of crossover from Usenet. High probability of plord-culpability.
I never read alt.fan.warlord. I do, however, have a 1994 copy from alt.config which begins as follows:

Good idea. Paul was a dead end-- he imported it to JMU from talk.bizarre. He found one on t.b from '92 here (http://groups.google.com/groups?q=skinner+box+clueless+talk+bizarre+greatly+misunderstood&hl=en&lr=&ie=UTF-8&oe=UTF-8&selm=6943%40ac13.cs.nps.navy.mil&rnum=10).
