Wow... We used to use (a permutation of (http://www.bbchatter.org/archive/files/reply.form.html)) that form on bb.chatter on the VAX at JMU. We called it the "Standard Reply Form," or SRF.
It *has* to be older than that, because I'm sure it was used @JMU longer than 1992, as 1) We used a permutation of the form, and 2) I don't know anyone who read a.f.w. I bet dwenius/plord could speak to this.
I never read alt.fan.warlord. I do, however, have a 1994 copy from alt.config which begins as follows:
